 Activities Sample Tool

  • Person-centered tool designed to gather information about interests and preferred activities during the behavior assessment process. Activities in this inventory tool may be ones the individual currently engages in or has in the past. They may also be ones they would like to learn or do in the future.

 Behavior Data Tool

  • This data tool uses the “Competing Behavior Pathway” as a format for recording difficult behavior occurrences. The Behavior Consultant may give it to caregivers to obtain consistent recording for the target behavior, setting events, and outcomes.

  • This data tool counts behavior occurrences in a time chart to pinpoint any particular time of day where difficult behavior is most likely to occur and not occur.

 Client Interview Tool

  • Tool designed to gather information directly from the individual through interviews. Some individuals will be able to answer the questions while other individuals may have dementia, cognitive impairments, or mental health issues where their verbal answers will reflect a unique view of the world, i.e. delusional, unrealistic, as if the person is living in the past, etc. This information is also relevant to determine what is important to the individual. In addition, information about how well the individual communicates with those around them can be obtained.

  • Data tool designed to analyze available incident reports concerning the individual the Behavior Assessment is conducted for.
