Individual Support Plans


Planning with people is an important part of the work that occurs to pOregon ISProvide services that help people grow, realize new dreams, and be valued members of their community. OTAC is contracted through the Office of Developmental Disability Services to provide limited training and technical assistance on the Oregon ISP process through January 2016.

For information about the former 24-hour ISP process (for people who live in 24-hour residential group homes), click here. These ISP materials are only used until the person’s next ISP comes due. At that time, their ISP will be renewed using the new Oregon ISP process.
