The PCP/ISP Team at OTAC has pub­lished sev­eral resources that may help you brush up your own ISP skills or train oth­ers on the required ISP process. For assis­tance with any of these mate­ri­als, please sub­mit a sup­port request.

Resource Descrip­tion
ISP Graphic Overview
New for 9.9.09, this is a road map style graphic overview of the flow of the ISP process. This is used at the begin­ning of ISP process train­ings to ori­ent new users to the process.
Adult Sample Forms
This is a par­tially com­plete ISP that is use­ful for see­ing how the ISP forms might look when they are filled out.
Children Sample Forms
This is a par­tially com­plete ISP that is use­ful for see­ing how the ISP forms might look when they are filled out.
Annotated Forms This is a copy of most of the ISP forms with notes about key points that are applic­a­ble to each page. This is great as a refresher tool. Check these out if you just want to refresh your mem­ory on the key required points for a spe­cific form.

This train­ing hand­out is used to dis­cuss who attends the Pre-Meeting and what must be done dur­ing the Pre-Meeting.
Making Changes Graphic
This hand­out sum­ma­rizes the key points of how to make changes through­out the ISP process.
